Prior to your first visit, you must fill out the new patient intake forms and policies below.

Credit Card on File

A valid credit or debit card is required to be kept on file. By providing a card, you authorize TAO: Center for Vitality to charge the card on file for amounts you are responsible for, including appointment fees, cancellation/no-show fees, products or services purchased, and any outstanding patient balance. You will receive a receipt for any charge made to your card.

How To Get Telemedicine Online Care

1. Telehealth appointments must be done on a laptop or desktop. Appointments done on cell phones run the risk of poor connection and we cannot be responsible for lost connection.
